About The Position

Monarch Communities is seeking compassionate individuals to join their team as Care Managers. This role involves providing personal care services, support, and companionship to residents, assisting with activities of daily living, and updating resident care plans based on observed changes. Requirements

• A positive attitude and enjoyment of working with people.
• High school diploma or equivalent.
• Current certification as a CNA/HHA.
• Knowledge and experience in working with the elderly population.
• Ability to stand, walk, sit, use fine and gross motor skills, reach with hands and arms, balance, stoop, kneel, crouch, talk, hear, and smell.
• Ability to lift or carry weight up to 50 lbs.
• Ability to use proper body mechanics for lifting and supporting residents.

• Dementia experience.

Responsibilities

• Provide warm, engaging personal care services, support, and companionship to residents.
• Engage and assist residents with activities of daily living (ADLs).
• Observe residents for changes in their day and personal care needs to update their personal Resident Care Plan.
• Encourage resident participation in community programs and activities.
