Seeking a Florida-licensed behavioral health professional, such as an LCSW, LMSW, APSW, CHES, LPC, LPCC, or LMFT, with experience managing complex behavioral health populations within the Medicaid environment. The ideal candidate will have a strong background in case management and care coordination for members with serious mental illness (SMI) and substance use disorder (SUD), ensuring seamless transitions of care across inpatient, outpatient, and community-based settings. This role requires expertise in multidisciplinary collaboration, member advocacy, crisis intervention, discharge planning, and connecting members to appropriate behavioral health and community resources to improve outcomes and reduce readmissions. The position includes approximately 10% local travel for member visits, provider collaboration, and care coordination activities. Job Summary Provides support for care management/care coordination activities and collaborates with multidisciplinary team coordinating integrated delivery of member care across the continuum. Strives to ensure member progress toward desired outcomes and contributes to overarching strategy to provide quality and cost-effective member care.
