About The Position

We are actively seeking motivated professionals to support care management activities and ensure seamless delivery of healthcare services. This role focuses on member outreach, care coordination, and enhancing member satisfaction through consistent engagement and documentation. This is a Contract position based out of Austin, TX. This is a fully remote position.

Requirements

  • High School Diploma or GED
  • 1–2 years of relevant experience in healthcare, customer service, or care coordination
  • Strong communication and documentation skills
  • Ability to work independently in a remote environment
  • Critical thinking and problem-solving abilities

Responsibilities

  • Conduct telephonic outreach and/or home visits to engage members and discuss care plans, next steps, and available resources
  • Coordinate care activities with healthcare providers, community partners, and caregivers
  • Serve as a primary support contact for member/provider inquiries related to care plans
  • Communicate with care managers and practitioners to ensure continuity of care
  • Complete health screenings and service assessments for new members
  • Maintain accurate and compliant documentation of member records
  • Identify member needs and provide referrals to care managers, disease management teams, or community resources
  • Educate members on benefits and available services
  • Adhere to all regulatory, contractual, and organizational standards
  • Conduct initial telephonic outreach for new members within 10 business days
  • Complete health screenings, documentation, and worklist creation
  • Support additional projects based on organizational needs
