About The Position

Humana Healthy Horizons in Virginia is looking for Care Management Support Assistants to support the administration of care management. This virtual team performs moderately complex administrative support tasks for the Commonwealth of Virginia Medicaid program. The role involves contributing to the administration of care management by managing shared mailboxes, entering data into work queues, mailing letters/educational materials, researching unable-to-contact members, and completing other administrative tasks as assigned. A key responsibility is ensuring the correct submission of Commonwealth-required deliverables. Additionally, the role includes conducting research to find alternative contact information for hard-to-search members to engage them in care management programs and providing support for inbound call escalations. The position offers non-clinical support for the assessment and evaluation of members' needs and requirements to achieve optimal wellness, working collaboratively with Coordinators (Care Coaches) and Field Care Managers. Daily tasks include varied clerical activities such as reporting data, tracking data, pulling reports, and monitoring/managing email, as well as performing computations. The role typically involves semi-routine assignments that are subject to change. Communication and follow-up with care management are essential.

Requirements

  • Candidate must reside in Virginia
  • 3 years of technical experience with Microsoft Word, Excel, Outlook
  • Familiarity with care and well-being resources
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences

Responsibilities
