The Care Management Supervisor (RN) provides clinical leadership and operational oversight to Care Managers/Care Coordinators to ensure high-quality, member-centered care management. This role safeguards compliance with regulatory requirements and VillageCare policies while reinforcing best practices in care coordination, utilization management, and quality improvement.
Requirements
- Bachelor's Degree
- Active New York State RN License
- Minimum 1 year of job-related experience
Responsibilities
- Supervising staff performance and caseload distribution
- Leading huddles, case conferences, chart reviews, and coaching
- Reviewing assessments/reassessments and ensuring appropriate plans of care
- Supporting interdisciplinary care team meetings and clinical rounds
- Ensuring accurate documentation of care plans, complaints/appeals, and member records
- Processing service authorizations within guidelines and coordinating covered/non-covered service linkages
- Participating in QA/PI activities, audits, and corrective action planning
- Serving as an escalation point for complex cases and transitions of care
