About The Position

BJC is looking for a Care Management Specialist to join their pharmacy team and collaborate with the chronic disease management team. This role involves referral management, scheduling, and communication with providers, specialists, and patients. A Pharmacy Tech license is required for this position. The work schedule is Monday-Friday, 8am-4:30pm. Barnes-Jewish Hospital at Washington University Medical Center is a leading hospital in Missouri and nationally recognized. The integrated practice model includes unit-based positions, access to extensive drug information resources, and an on-site drug information center for support.

Requirements

  • Pharmacy Tech license
  • High School Diploma or GED
  • 2-5 years of experience

Responsibilities

  • Serves as the primary resource between the care management team, Providers and other patient care entities on projects that impact the care management of patients in the ACO (Accountable Care Organization).
  • Responsible for collecting, developing and monitoring project documentation to implement and sustain patient-centered care activities.
  • Collaborates with directors, managers and other disciplines to plan and implement projects related to patient care area.
  • Assembles project plans and teamwork assignments, directing and monitoring work efforts on a daily basis, identifying resource needs, performing quality review, and escalating functional, quality, timeline issues appropriately.
  • Ensures overall coordination, status reporting and stability of projects or operations-oriented work efforts.
  • Establishes methods to monitor the availability of reportable data and assists with planning and implementing new projects.
  • Works directly with leadership on internal business development activities, integration of business and quality goals and objectives, scope of services, and compliance planning.
