Requirements
- Master’s Degree in Social Work from an accredited social work program
- 1 year of experience in an acute or post-acute setting
Nice To Haves
- LMSW - Licensed Masters Social Work
- Accredited Case Manager (ACM)
- CCM - Certified Case Manager
Responsibilities
- Promote patient and family wellness, improved care outcomes, access to appropriate hospital and community resources.
- Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments.
- Promote quality of care and effective transitions planning through collaboration with all service team members, physicians, patients, external agencies and families.
