About The Position

Collaborates with clinic leadership, physicians and staff to implement care management and quality improvement functions to optimize outcomes. This may include distribution and discussion of data and reports with care providers, staff education, and gathering and compiling input from physicians and other care providers. Receives and distributes reports showing the progress and implementation of care management initiatives within market and clinics. Assists leadership with identification and implementation of improvement strategies as designated. Follows up with patients based on identified physician care plan needs through various methods to include phone calls, letters, and face-to-face meetings. Acts as a resource to staff on current and new care management initiatives. Works closely with clinic staff to assess and improve care management results through discussion and staff education. May participate in medical record reviews as necessary Reviews data to identify clinic and market trends. May presents data as requested for the purpose of creating action plans needed to improve patient care. Acts as an informational resource to care management administration. Adheres to Medical Assistant scope of practice, follows all policies and procedures, and maintains training and competency based on area of specialty when providing patient care.

Requirements

  • Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 45 days unless department leader has determined it is not required.
  • Certified or registered as a CMA or RMA by one of the following National Organizations: American Association of Medical Assistants (AAMA), Certified Clinical Medical Assistant (CCMA) certification issued by the National Health Career Association (NHA), RMA certification issued by the American Medical Technologists (AMT), Medical Assistant (NCMA) certification issued by the National Center for Competency Testing (NCCT), Clinical Medical Assistant Certification (CMAC), or American Medical Certification Association (AMCA).
  • High School Diploma or GED required
  • Associate's Degree (or equivalent knowledge) in Allied Health.
  • Completion of an accredited or approved program in Medical Assistant, or 2 years of verifiable full-time experience as a Medical Assistant within the last 5 years.
  • Typically requires 2 years of experience in an ambulatory setting that includes experiences in direct patient care, general knowledge of chronic disease states, providing staff training, and assisting with gathering, interpreting and distributing data.
  • Excellent oral and written communication skills.
  • Demonstrated ability to effectively communicate with multi-discipline team members and other professionals.
  • Proficient in the use of Microsoft office suite or similar products, electronic mail and electronic medical records systems.
  • Ability to work independently with minimal supervision.
  • Excellent attention to detail with the ability to prioritize and organize data / projects effectively.
