Northwest Regional Council (NWRC) is a multi-service agency that covers the Washington counties of Island, San Juan, Skagit and Whatcom, with offices in Bellingham and Mount Vernon, Washington. Due to an upcoming retirement, we are seeking a Care Management Program Supervisor to oversee our Case Management team in the Bellingham office. Long-term care case management provides older adults and adults with disabilities access to community-based long-term care services. Case managers assess their client’s health and care needs and authorize payment for services to help people maximize their health and well-being and maintain independence at home. The Care Management Program Supervisor is responsible for coordinating and overseeing the NWRC Care Management Programs. They provide direct supervision, coaching, leadership, and training to staff, while emphasizing clients’ preferred choices. The supervisor promotes collaboration among NWRC team members, as well as maintaining relationships with community partners and key personnel at the Home and Community Living Administration. A culture of caring, knowledge sharing, and empowerment is fostered to encourage staff independence. The supervisor assists their team by evaluating suitable in-home long-term care programs that align with the clients' preferred living arrangements and desired services. Supervisors are also tasked with actively engaging in their own education regarding various programs, databases, and payment systems to ensure they possess sufficient knowledge to support staff, to deliver individual, team, and regional team trainings, to develop internal policies and procedures, and edit them as necessary. The supervisor frequently collaborates with other members of the case management leadership team to resolve departmental issues, identify necessary staff training, and workforce planning.
