Appalachian Community Services provides mental health, behavioral health, substance use, and developmental disability services across seven western North Carolina counties. The Care Management Professional proactively engages high-risk members, particularly those with behavioral health challenges, and supports their whole-person health. The role assesses, evaluates, and coordinates members’ physical, behavioral, and support needs, helping identify and resolve barriers to effective care. Responsibilities include assessing and updating Individualized Service Plans (ISPs), supporting medication management, and providing self-management tools beyond the counseling and guidance members receive during provider visits. The Care Manager facilitates communication among members’ providers—including doctors, nurses, and specialists—to coordinate their care. The position requires handling a high volume of inbound and outbound calls, engaging hard-to-reach members, building rapport with people with complex needs, and meeting established productivity, quality, and performance expectations. The organization describes a supportive clinical and operational leadership structure, streamlined onboarding and credentialing, opportunities to build preferred clinical focus areas, and flexibility to shape a schedule while maintaining a steady caseload. Compensation is $25–$32.10 per hour, with placement based on licensure, education, and experience.
