About The Position
The Behavioral Health and Intellectual/Developmental Disability (I/DD) Tailored Care Management (TCM) model reflects the goal of whole-person care management in NC Medicaid Managed Care. Provider-based care management promotes integrated care and offers beneficiaries a choice in how they receive care management. This role involves developing care management comprehensive assessments and care plans/individual support plans with beneficiaries, coordinating/referring/monitoring all services (medical, pharmacy, behavioral health, waiver services, food, housing, transportation, community resource supports), supporting beneficiaries in a crisis (with planning supports), arranging for annual physicals, Innovations and TBI waiver care coordination (if applicable), and convening/consulting with a multidisciplinary care team. This description outlines the general scope and level of work for this role and is not intended to include every duty or responsibility that may be required of employees in this position.
Requirements
- A bachelor's degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area, or licensure as a registered nurse (RN)
- Two years of experience working directly with individuals with behavioral health conditions (if serving members with behavioral health needs) or with I/DD or a TBI condition(s)
- For Care managers serving members with LTSS needs: two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience, in addition to the requirements cited above.
- A valid driver's license, auto insurance, and reliable transportation
- Must be authorized to work in the United States.
Responsibilities
- Maintain accurate and timely clinical documentation
- Collaborate with care teams as needed
- Maintain compliance with all clinical and regulatory standards
- Participate in supervision (if applicable based on licensure level)
