About The Position
The Care Management Extender will provide programmatic support services for the Care Management department covering in UH, Ross, Brain and Spine, East Hospital and Dodd Hall. The Care Management Extender will be responsible for coordinating inclusive of throughput/capacity management, discharge planning, and delegated functions at the direction of the Care Manager and/or Social Worker.
