About The Position

The Care Management Coordinator provides support to the pharmacist-led chronic disease management program and assists with chronic disease management and medication therapy management. The Coordinator Care Management reviews registry data and other data sources to determine, based on guidelines, which patients are eligible for chronic disease management and performs outreach to enroll appropriate patients in the program.

Requirements

• High School diploma.
• Pharmacy technician certification preferred.
• 2 years of clinical experience preferred.
• Bilingual English/Spanish preferred.

Responsibilities

• Provide support to the pharmacist-led chronic disease management program.
• Assist with chronic disease management and medication therapy management.
• Review registry data and other data sources to determine patient eligibility for chronic disease management based on guidelines.
• Perform outreach to enroll appropriate patients in the program.
