About The Position

The Care Management Coordinator provides essential clerical support for the Care Management Department. Under the supervision of licensed clinical staff, accountabilities include coordinating communication between patients, health care providers, insurance companies and other stakeholders to ensure safe and efficient transitions of care, payment for clinical services provided and regulatory compliance. The goal of the Care Management Coordinator is to ensure the maintenance of accurate records, process documentation and support the delivery of quality patient care. This role requires excellent organizational skills, attention to detail, and the ability to handle multiple tasks simultaneously.

Requirements

  • 2 years of administrative experience, preferably in a healthcare setting or demonstrated knowledge of healthcare terminology
  • Excellent organizational skills

Nice To Haves

  • Bachelor's Degree or higher in Business, Healthcare, or related field
