Position Summary The role of the Care Management Associate (CMA) is to support Health Coaches and Nurses, as well as members participating in the Healthy Aging Support Program (HASP). The CMA manages group coaching attendance, schedules appointments, places reminder calls for upcoming appointments and assists members with technical needs related to the group coaching platform. Job role responsibilities also include data entry for members enrolled in HASP.

Requirements

  • Candidate must be willing to support 10 AM – 7 PM EST hours
  • Dependents must have separate care arrangements during work hours, as continuous care responsibilities during shift times are not permitted
  • Access to a private, dedicated space to conduct work effectively to meet the requirements of the position
  • 1 + year(s) of customer service experience, preferably in the health care industry
  • Experience with computers including knowledge of Microsoft Outlook and Excel
  • Strong organizational skills, including effective verbal, telephonic, and written communication

  • Experience with data entry and documentation within member records
  • Bilingual (Spanish)

Responsibilities

  • Effectively supports Health Coaches during each group coaching session, responding promptly when assistance is needed
  • Monitors and responds to Healthy Aging Support Program voicemails
  • Accurately and consistently documents in member record per workflow
  • Protects the confidentiality of member information and adheres to company privacy policies
  • Ability to be flexible, managing multiple priorities and adapting to change with enthusiasm
  • Determined to build strong relationships with peers
