Requirements

1-2 years of experience as a medical assistant, office assistant, or adjacent role.
1-2 years of experience in a call center environment.
Must reside in Arizona.
High school diploma or GED.
Computer literacy to navigate through internal/external computer systems, including Microsoft Excel and Microsoft Word.

Nice To Haves

Experience working in managed care.
Effective communication, telephonic, and organization skills.
Familiarity with basic medical terminology and concepts used in care management.
Strong customer service skills to coordinate service delivery, including attention to customers, sensitivity to issues, proactive identification, and resolution of issues to promote positive outcomes for members.

Responsibilities

Initiates contact with members within three days of discharge from a hospital or facility.
Ensures outreach is focused on discharge planning and scheduling follow-up visits with the member’s assigned Primary Care Physician (PCP).
Applies critical thinking and clinical judgment to support the case management process.
Collaborates with internal teams and external providers to facilitate appropriate healthcare outcomes.
Uses care management tools and resources to educate and support members.
Promotes and supports the quality administration of healthcare services.
Ensures that all actions align with organizational standards and regulatory requirements.
