About The Position

Assists the RN Care Coordinator in the follow up regarding information in the care plan, service auths, and the submission of data in to the multiple different health plan sites along with the Department of Health and Human Services sites. As well as assists in the reinforcement of the goals, completed by the RN Care Coordinator, and the collaboration of care with the care team for that member.

Requirements

• Graduate of LPN, Medical Secretary, Health Unit Coordinator, Medical Assistant, Paramedic, EMT or similar program.
• If no education, 2 years clinical scheduling experience.
