About The Position

InnovaCare Management Services Company, LLC is seeking a Care Management Assistant to provide administrative support to case managers and the clinic team. This role involves reviewing daily schedules to identify high-cost patients, participating in daily clinic huddles and weekly UM/CM rounds, and performing task-driven outreach according to the plan of care. The assistant will also schedule follow-up appointments after ED events, send reminders for patients not seen or not established, and follow up with other teams for pending tasks. Additionally, the role includes providing Tier Two patient outreach for short-term case management, following up on referral/DME/specialist visit completeness with patients, and assisting with required paperwork from hospitals, SNFs, LTACs, and home health. The assistant will also conduct "tuck in" calls to real-time rising risk patients and assist case managers and clinic staff with daily outreach for specific tasks, including confirming medication adherence, specialist appointment attendance, and the receipt of DME, HHC, or other resources outlined in the plan of care.

Requirements

  • College degree or completion of college courses in healthcare, care management, or similar focus.
  • Medical Assistant, health coaching, or care management prior experience preferred

Responsibilities
