About The Position

The Care Management Assistant coordinates administrative, operational, and customer support activities that support care management programs and healthcare services. You will perform advanced administrative/operational/customer support responsibilities that require independent initiative and judgment. You will report to the Associate Director, Consumer Experience. You will support the assessment, coordination, and facilitation of members' care needs to help achieve and maintain health and wellness outcomes. You will collaborate with members, providers, caregivers, and care teams to support member wellbeing through care coordination, patient record review and maintenance, and the preparation of care summaries that inform care teams, guide next steps, and improve patient outcomes. You will use discretion and judgment in prioritizing requests, interpreting and adapting procedures, and applying established techniques. You will use prior experience of care management processes, healthcare operations, and organizational practices to perform the role.

Requirements

  • High School Diploma or GED.
  • Experience providing in-center or virtual support or interaction with patients.
  • Experience in patient support, care coordination, or a related healthcare setting.
  • Experience with data entry and maintaining information within an electronic medical record (EMR) system.
  • Can work EST or CST.
