The Care Extender / Peer Support Specialist - Foster Care is responsible for engaging with community partners and builds strategy with local stakeholders; providing education, strengthening relationships, and helping resolve case or broader systems issues. Supports care coordination teams and connect members and caregivers with those who can provide insight and expertise to assist in overcoming challenges. This position is primarily virtual/working from your home office, with responsibilities that include community-based travel and in-home member visits to support member needs. This role requires associates to be in-office 1 - 2 days per week, fostering collaboration and connectivity, while providing flexibility to support productivity and work-life balance. This approach combines structured office engagement with the autonomy of virtual work, promoting a dynamic and adaptable workplace.
