Care Coordinator - Broomfield Adult Health Home
About The Position
This position is for a Care Coordinator at Broomfield Adult Health Home. It is an opportunity for individuals with a Bachelor's degree in Psychology, Social Work, or Sociology to gain experience in mental health settings. The role involves helping clients focus on health and wellness treatment goals, working with a vulnerable client population, and developing case management and counseling skills. Requirements
- Bachelor's Degree in Psychology, Social Work, Counseling or a related field OR one-year experience in the behavioral health field for non-related Bachelor's degrees
- Experience working with adults in an outpatient setting
- Interest in working with adults with mental illness and substance use issues
- Experience with clinical modalities: Motivational Interviewing (MI) and Dialectical Behavior Therapy (DBT), other Cognitive Behavior Therapies (CBT)
- Collaborative and client centered orientation
- Collaborative team member
- Familiarity with insurance programs like Medicare and Medicaid
