About The Position

The Care Coordinator is responsible for providing and coordinating resident care to ensure quality care is delivered. This role also involves providing care team leadership. The position requires the individual to be responsible for the safety of residents in the case of emergency and to serve as a 'go-to' resource person for team members. The Care Coordinator will participate in the orientation and training for care team members, foster a positive work environment, and support the development of skills with other care team members. Responsibilities include reviewing and updating resident care as outlined in resident service plans, performing rounds with physicians, and communicating new orders to staff and family as required. The role also involves assisting with schedule coverage needs, promptly answering resident call signals, door, and safety alarms, and administering/passing medications according to physician orders and policies. Additionally, the Care Coordinator will review and process physician orders using eMAR, assist with resident evaluations, conduct departmental reviews/audits, and provide results to the Director of Resident Care. Ensuring adequate medication and supply inventory for residents, observing and reporting non-emergent resident behavior and condition changes, and completing daily charting, documentation, and reporting are also key functions. The Care Coordinator will participate in or lead activities as planned by Life Enrichment and assist/remind residents to participate. Compliance with infection control, safety, emergency, and confidentiality policies and procedures is mandatory. Participation in the on-call rotation as assigned, promoting positive relationships with residents, staff, and visitors, and remaining awake and alert while on duty are expected. Any other duties as requested within the scope of responsibility and requirements of the job will also be performed.

Requirements

  • High school diploma/GED
  • Successful completion of Care Specialist I and II curriculum within 90 days of hire
  • At least 6 months of recent experience as a Care Specialist or equivalent experience
  • CPR/First Aid certification or obtain within thirty (30) days of hire
  • Demonstrable knowledge and experience using Microsoft Office in a Windows environment
  • Capacity to master company specific software
  • Ability to multi-task, prioritize and work with minimal supervision
  • Effective oral and written communication skills
  • Professional telephone etiquette and customer service skills
  • Superior communication and interpersonal skills
  • Communicate in a respectful and professional manner with residents, families, co-workers and leadership staff at the community
  • Developed organizational skills
  • Ability to problem solve and demonstrate good judgement

Responsibilities
