About The Position

The Care Coordinator is responsible for assessing consumer needs and strengths, identifying goals, and developing Individualized Service Plans (ISPs). This role involves linking consumers to community resources, monitoring progress, and advocating on their behalf. The Care Coordinator collaborates with treatment teams and external sources, evaluates consumers for discharge planning, and demonstrates child/adolescent specific competencies. They are also responsible for seeking clinical supervision, maintaining timely and comprehensive documentation, and managing their caseload to meet productivity standards. Additionally, the role involves participating in performance improvement activities, committees, and adhering to company policies and procedures.

Requirements

  • Bachelor’s degree in a human services related field or social work program
  • Must be at least age 23
  • Maintain a valid verifiable driver license
  • Maintain a clear driving record
  • Knowledge in child/adolescent specialty, outlined below, to be attained within six (6) months of employment:
  • Knowledge of psychiatric disorders in the child/adolescent population
  • Basic knowledge of growth and development of child/adolescent consumers as related to all intellectual, emotional, spiritual, and physical aspects of their development
  • Knowledge of intellectual and developmental disabilities in the child/adolescent population
  • Basic knowledge of behavior management and parenting issues
  • Knowledge of cultural diversity
  • Knowledge of behavior management, crisis intervention and interventions common to acute psychotic consumers, as well as, to non-violent crisis intervention practice
  • Knowledge of effects of trauma including physical/sexual/emotional abuse and neglect issues
  • Knowledge of available community resources
  • Certified by the Alabama State Department of Mental Health to provide child/adolescent care coordination
  • Certified to administer the CANS (Child and Adolescent Needs and Strengths Assessment)

Responsibilities

  • Assess the consumer’s needs and strengths utilizing the CANS
  • Identify goals and plans for appropriate services based on the CANS
  • Develop Individualized Service Plan (ISP) within 30 of first face-to-face
  • Provide appropriate linkage to community resources
  • Provide ongoing monitoring of consumers’ progress and needs as assessed in the CANS
  • Advocate on behalf of the consumers to secure community resources
  • Collaborate with the treatment team members and outside sources in the delivery of care coordination
  • Evaluate the consumer for discharge planning
  • Demonstrate the application of child/adolescent specific competencies
  • Seek clinical supervision and consultation as needed
  • Accept and employ suggestions for improvement
  • Actively work to enhance knowledge and skills
  • Document in a timely fashion per AltaPointe policy
  • Document appropriate, complete, and comprehensive CANS assessments
  • Document measurable and specific goals in the individualized service plan
  • Document in a clear and concise manner, types of consumer problems, ISP goals addressed and services provided
  • Document case disposition, case closings, follow-up and referrals
  • Document legibly
  • Treat consumers with care, dignity and compassion
  • Respect consumer’s privacy and confidentiality
  • Be pleasant and cooperative with others
  • Assist consumers and visitors as needed
  • Be sensitive to the consumer’s needs, expectations and individual differences
  • Be gentle and calm to consumers and families
  • Effectively manage caseload based on consumer needs
  • Meet AltaPointe productivity standards
  • Actively participate in Performance Improvement activities
  • Actively participate in AltaPointe committees as required
  • Complete assigned tasks in a timely manner
  • Follow AltaPointe policies and procedures
  • Attend required in-service training and other workshops
