About The Position

The Care Coordinator provides direct services to both youth and adult individuals diagnosed with chronic, serious mental illness. Services are provided in the community and on-site. Responsibilities include assessing, planning, coordinating, monitoring, and linking patients to community resources and service providers. The Care Coordinator also provides skills building, psychosocial rehab and crisis intervention services. Other key responsibilities for this position consist of developing the recovery plan and completing the Adult Needs and Strengths Assessment (ANSA) or the Child and Adolescent Needs and Strengths (CANS) for patients on the assigned caseload. Documentation and assessments must meet all applicable standards and established performance benchmarks. The Care Coordinator performs other duties as assigned by the supervisor. Must be able to work within a multi-disciplinary treatment team. Must have excellent engagement, time management and organizational skills. Routine hours are 8:00 AM to 5 PM, Monday through Friday with flexibility for schedule changes to allow for appointments outside of routine hours. Must have reliable vehicle for transportation. The Care Coordinator reports directly to the Clinic Manager and is responsible and accountable for the adherence to and promotion of compliance policy.

Requirements

  • Bachelor’s or advanced degree from an accredited college or University in Counseling, educational psychology, or human service field, early childhood intervention, gerontology, human growth and development, medicine, nursing, physician assistant, Psychology, rehabilitation, social work, sociology and special education.
  • Texas driver’s license in good standing as defined by Texana Center’s procedure.
  • Ability to perform SAMA and CPR.
  • Ability to perform moderate lifting and carrying (15-44#), pushing, stooping, repeated bending, reaching above shoulder, simple grasping, hearing, walking, sitting and twisting.
  • Ability to see, write, operate office equipment, operate a motor vehicle and have depth perception.
  • Ability to perform job functions with or without reasonable accommodation.
  • Excellent engagement, time management and organizational skills.
  • Reliable vehicle for transportation.

Nice To Haves

  • 1 year of work experience providing direct services within a Behavioral Health or Social Service setting
