About The Position

The Behavioral Health Care Coordinator supports Curana Health’s behavioral health programs by providing proactive, non-clinical care coordination for patients in long-term care, senior living, and post-acute settings. This role is responsible for managing a defined patient caseload, ensuring regular patient engagement, coordinating care across interdisciplinary teams, and supporting providers in delivering high-quality behavioral health services. The Care Manager plays a key role in identifying changes in patient condition, reinforcing treatment plans, and facilitating communication between patients, families, and care teams. This is a remote, high-volume, metrics-driven role requiring strong organization, communication, and the ability to work independently while adhering to established workflows and documentation standards.

Requirements

  • Exhibits knowledge of pathophysiology and accepted treatment protocols for common behavioral health diagnoses.
  • Ability to analyze patient records to identify gaps in care and report to the provider.
  • Ability to work in a remote environment that is free of distractions.
  • Proficient computer skills and ability to adapt to various technology platforms.
  • Excellent written and verbal communication skills.
  • Demonstrated experience in the usage of clinical data to guide decision making.
  • Must have the ability to function independently and as a member of the interdisciplinary care team.
  • Must hold an active, unrestricted compact LPN or RN or MA license.
  • Ability to obtain additional state licenses, as needed
  • 2+ years of experience in healthcare required. Care settings may include inpatient, outpatient, or skilled nursing facilities.

Responsibilities

  • Manage an assigned caseload of patients requiring behavioral health support.
  • Conduct routine patient outreach and follow-ups to monitor status and needs.
  • Reinforce care and treatment plans, treatment adherence, and patient engagement.
  • Identify and escalate changes in condition, safety concerns, or barriers to care.
  • Communicate with patients, families, and caregivers as appropriate.
  • Partner with interdisciplinary teams including providers, nursing staff, social services, and facility leadership.
  • Support coordination of behavioral health services, including scheduling and follow-up care.
  • Facilitate communication to ensure continuity and alignment of care plans.
  • Accurately document all patient interactions and coordination activities in the appropriate systems.
  • Maintain timely and complete records to support program requirements and compliance standards.
  • Adhere to organizational policies, privacy standards, and regulatory requirements.
  • Other duties as assigned
