The Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized services including orientation, transportation, doctor visits, and scheduling. This role ensures programs and services are implemented at the highest standards and clients receive the highest level of care. The Care Coordinator will also be responsible for client supervision, facilitating groups, conducting client orientations, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, administering and submitting client Urine Analysis documentation in EMR, assisting with facility cleanliness, coordinating client lunch daily, and serving as a role model. The position requires maintaining the confidential nature of clients and related activities, complying with the dress code, completing tasks in a timely and accurate manner, and attending designated staff and in-service meetings. The Care Coordinator must be able to maintain clear boundaries with clients and staff, set limits, manage clients when in need of direct supervision, and maintain appropriate and professional interactions. All documentation in EMR must be submitted within 24 hours.
