The Care Coordinator is a central member of the on-site clinical team, helping ensure that patients with complex medical needs receive coordinated, responsive, and thoughtful care. Working closely with our Care Coordination Manager, Advanced Practice Provider, physicians, and facility team, you’ll help organize each patient’s care, anticipate needs, and make sure nothing falls through the cracks. You’ll build relationships with patients and families, coordinate appointments and transitions, prepare the clinical team for efficient visits, and follow through on the details that can make the difference between a patient remaining stable in the facility and ending up back in the hospital. This is an on-site, hourly role with a predictable Monday–Friday, 8:00 a.m.–4:00 p.m with significant flexibility to work remotely in the afternoons, with no evening or weekend requirements. It’s an ideal opportunity for an LPN or experienced Medical Assistant who enjoys patient relationships, thrives on organization and follow-through, and wants to play a meaningful role on a collaborative clinical team.
