Under the supervision of the Program Director, the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized services including orientation, transportation, doctor visits, and schedules. This role ensures programs and services are implemented at the highest standards and clients receive the highest level of care. The Care Coordinator is responsible for client supervision, ensuring they attend designated groups, and facilitating assigned groups and activities. They will conduct client orientation for all admits, including UA screening, PHQ9, and other assessments. The role also involves ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, and driving clients as needed. All transportation and group activity must be documented in EMR, along with client Urine Analysis administration and submission. The Care Coordinator will also assist with facility cleanliness, coordinate client lunch daily, and serve as a role model through a positive, caring, and professional attitude. Maintaining client confidentiality, complying with the dress code, completing tasks accurately and timely, and attending staff meetings are also key responsibilities. The ability to maintain clear boundaries, set limits, and manage clients needing direct supervision is essential, as is maintaining appropriate and professional interactions. All EMR documentation must be submitted within 24 hours.
