Under the supervision of the Program Director, the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized services including orientation, transportation, doctor visits, and schedules. This role ensures programs and services are implemented at the highest standards and clients receive the highest level of care. The Care Coordinator is also responsible for client supervision, ensuring they attend designated groups, facilitating assigned groups and activities, and conducting client orientation. They must ensure compliance with Joint Commission Standards, document shift reports, coordinate transportation schedules, and administer client Urine Analysis. Additionally, the role involves assisting with facility cleanliness, coordinating client lunch, serving as a role model, maintaining confidentiality, and completing tasks in a timely and accurate manner. The Care Coordinator must also maintain clear boundaries with clients and staff, set limits, and manage clients when direct supervision is needed, ensuring all documentation in EMR is submitted within 24 hours.
