About The Position

Bluestone is hiring a Care Coordinator to support Minnesota’s senior population in navigating healthcare and living healthier, more independent lives. The role manages Department of Human Services (DHS) and Centers for Medicare & Medicaid Services (CMS) requirements for MSHO/MSC+ members within an assigned geographic area. It is hybrid, combining work from home with local visits to members’ homes and assisted living facilities in the Minneapolis and Hennepin County areas. The Care Coordinator conducts face-to-face visits and assessments, manages the Elderly Waiver, develops individualized care plans, and connects members with medical care, waiver services, and community resources. The role collaborates with medical care teams to support quality and value-based care, and helps members through transitions of care and safe discharges. The schedule is full-time: Monday through Thursday, 8:00 AM–5:00 PM, and Friday, 8:00 AM–3:00 PM. There are no evening, weekend, holiday, or on-call requirements. The listed salary range is $65,000–$75,000, commensurate with experience. A $2,500 sign-on bonus is paid in two installments: $1,000 after 90 days and $1,500 at the one-year anniversary. Bluestone provides a six-month onboarding program, preceptor support, ongoing mentoring and coaching, continuing education, professional development, collaborative managers, and documentation auditors focused on education and quality.
