About The Position

ThriveKids Student Wellness is a community-based program at Children’s Hospital New Orleans with a mission to improve healthcare and academic outcomes for students by partnering with schools to expand access to healthcare services. This role involves staffing schools with healthcare professionals and providing/facilitating direct patient care services. The Care Coordinator will work as a member of the ThriveKids Care Coordination Team, providing care coordination services to youth enrolled in Thrivekids. They will have access to Children’s Hospital New Orleans specialty and behavioral health services to supplement and resource student health needs. The coordinator will also assist in developing a comprehensive guide to external/community behavioral health resources for families. This is a full-time direct employee position with Children’s Hospital New Orleans, offering the option of a 10-month or 12-month working schedule (compensation adjusted accordingly). The role serves as a communication liaison between medical providers, families, and school personnel, and facilitates re-entry meetings for students returning after extended absences due to crisis or hospitalization. Insights gained will contribute to ThriveKids behavioral health programmatic development. The position offers opportunities for career development and the option to provide student/patient counseling as needed. Travel to individual schools may be required.

Requirements

  • Master’s degree from an accredited school in Social Work.
  • Licensed in the State of Louisiana.
  • Experience or training in providing services to children and families.
  • Excellent skills in communication.
  • Ability to work both independently and in a team environment.
  • Ability to prioritize.
  • Clinical knowledge in order to assess the needs of youth enrolled in Thrivekids.
  • LMSW in clinical supervision.
  • Maintains current driver's license and car insurance.
  • Working knowledge of federal and state laws, policies and regulations relevant to schools.
  • Record of high performance in challenging situations, requiring problem solving skills and good judgement.
  • Good time management, communication (oral and written), conflict management, organizational and planning skills.
  • Ability to maintain key community partnerships through excellent people skills, demonstration of accountability, and self-awareness.
  • Ability to work collaboratively with other professionals to obtain positive solutions.
  • Strong critical thinking skills.
  • Willingness to contribute to program development.

Responsibilities
