Under the supervision of the Program Director, the Care Coordinator is responsible for the coordination of care for all ROWI clients. This role involves coordinating individualized services including orientation, transportation, doctor visits, and scheduling. The Care Coordinator ensures programs and services are implemented at the highest standards and clients receive the highest level of care. They are responsible for client supervision, facilitating assigned groups and activities, conducting client orientations, and ensuring compliance with Joint Commission Standards. This position also involves documenting shift reports, coordinating transportation schedules, administering client Urine Analysis, and assisting with daily lunch coordination. The Care Coordinator serves as a role model, maintains client confidentiality, and completes tasks accurately and in a timely manner. They must also maintain clear boundaries with clients and staff, and ensure all EMR documentation is submitted within 24 hours.
