About The Position

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator for the OhioRISE program in Highland County, OH. ISBH is a community-minded, forward-thinking behavioral health organization dedicated to helping individuals on their path to health and well-being. We provide services in homes and communities, connecting individuals with necessary resources across Southeastern and Central Ohio. Our approach is collaborative and personalized, aiming to promote healthy people and strong communities. The OhioRISE program specifically supports youth aged 0-20 with complex behavioral health and/or substance use needs through a Medicaid managed care program. The Care Coordinator is responsible for building and leading a team, as defined by the family, to support youth in their homes, schools, and communities. The goal is to keep families together and minimize hospitalizations. This involves collaborating with social service providers, community partners, and the family's support systems. The Care Coordinator facilitates team meetings to identify needs, set goals, and develop action plans to achieve family objectives, ensuring the youth and family have a voice and choice in all coordinated care and services.

Requirements

  • Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field is required (three years with a high school diploma or equivalent; or two years with an associate degree or bachelor's degree; or one year with a master's degree or higher).
  • Experience in one or more of the following areas is required: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or, child development.
  • Have the qualifications and experience needed to work with children and families who are experiencing serious emotional disturbance (SED), trauma, co-occurring behavioral health disorders and who are engaged with one or more child serving systems (e.g. child welfare, intellectual and developmental disabilities, juvenile justice, education) is required.
  • Able to effectively communicate through verbal/written expression required.
  • Ability to use a personal vehicle or company vehicle to provide services in homes, schools, and community settings is required.
  • Ability to maintain confidential information is required.
  • Ability to adhere to all safety, rules, regulations and requirements is required.
  • Ability to bill Medicaid without restrictions is required.
  • A valid driver's license and the ability to operate a motor vehicle is required.
  • An appropriate level of auto insurance coverage is required.
  • Ability to manage deadlines within a fast-paced, high-volume environment is required.
  • Ability to operate in an Internet-based, automated office environment is required.
  • Ability to operate, receive and conduct on-camera video calls and/or meetings in an automated office environment is required.
  • Ability to maintain a minimum of 100mbps for high-speed internet connection is required.
  • Prolonged periods of sitting at a desk and working on a computer is required.
  • Must be able to lift up to 15 pounds at times is required.

Nice To Haves

  • Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).
  • Two years of experience in a coordinated supportive services or care coordination role that required high efficiency, organization, and attention to detail is preferred.
  • Experience working with people with autism spectrum disorders and developmental disabilities is preferred.

Responsibilities

  • Uses a strengths-based and solution-focused mindset in all job functions and interactions internally and externally.
  • Develop, lead, and facilitate team meetings for the young person and their identified team.
  • Provides care coordination within the family’s home, community, or preferred location.
  • Partners along with the young person, family, natural supports, and other service providers to identify care coordination needs/services to ensure the best outcomes for young people and families.
  • Maintains required documentation to access and maintain OhioRISE benefits.
  • Meets quality and fidelity requirements as defined by ISBH OhioRISE standards.
  • Continually acquires and maintains knowledge of community resources.
  • Attends mandatory in-person and virtual trainings and supervisions which will require travel.
  • Completes all assigned trainings within identified timelines and maintains required certifications.
  • Follows ISBH policies and procedures in maintaining confidentiality, professionalism, and incident reporting.
  • Utilizes databases and Electronic Health Record systems for accurate client documentation.
  • Maintain compliance with HIPAA, mandated reporting requirements, and professional ethical guidelines.
  • Travel within assigned service area.
  • All other duties as assigned.
