About The Position

ACCORD is seeking a Part-Time Care Coordinator to serve clients with disabilities, mental health conditions, or terminal illnesses in St. Louis County or surrounding counties. This role involves providing care coordination and case management using person-centered practices to coordinate services among various professionals and settings. The position is part-time, requiring 20 hours per week.

Requirements

  • Must live in St. Louis County or surrounding counties.
  • Bachelor's or Master’s degree in social work and licensed by the Minnesota Board of Social Work.
  • Registered Nurse licensed by the Minnesota Board of Nursing.
  • Minimum of 2 years of work experience in the field of social services, preferably working with older adults and/or people living with disabilities.
  • Knowledge of health, social services, and funding sources that support individuals.
  • Self-starter with ability to work independently and as part of a team.
  • Strong organizational skills with ability to work in a fast-paced environment.
  • Proficient with technology and demonstrated ability to manage multiple computer systems.

Responsibilities

  • Provide care coordination services to individuals of all ages with disabilities, mental health conditions, or terminal illness.
  • Using person-centered practices, coordinate services for individuals with disabilities among different health and social service professionals and across settings of care.
  • Interact professionally with interdisciplinary team members including the person supported, family caregivers and guardians, employers, case managers, community providers, and other involved stakeholders.
  • Meet remotely and face-to-face with individuals and their support teams to assess physical and mental wellness, needs, preferences, and abilities; apply information to develop tailored treatment plans.
  • Integrate and improve the coordination of care by ensuring optimal health status, decreasing health decline, providing social or community support systems, promoting a safe environment, reducing the impact of behavioral health issues, and encouraging self-reliance.
  • Provide Care Coordination services to assess, plan, facilitate and advocate for options and services to meet an individual’s health needs, through communication and available resources to promote high-quality, cost-effective health outcomes.
  • Advocate for individuals, including awareness of and sensitivity to culturally appropriate care.
  • Apply ethical practice principles such as respect for the autonomy, dignity, privacy and rights of the individual.
  • Promote collaborative, proactive, and person-focused relationships.
  • Provide timely and beneficial services that promote quality and cost-effective health outcomes.
  • Manage a flexible caseload of up to 60-80 members.
  • Attend all required annual and other meetings, demonstrating a high level of professionalism and genuine care for the person supported.
  • Practice customer service excellence and prompt responses (within one business day) for all communication, including written and verbal correspondence.
  • Demonstrate strong written communication skills; complete all required paperwork to comply with contractors and funding sources.
  • Complete documentation of all services provided in the selected electronic health record/database system within one business day of providing services.
  • Meet compliance standards set forth by the managed care organizations standards.
  • As a mandated reporter, respond and report all incidents that occur while providing services to protect the health and safety of and minimize risk of harm to the person(s) supported following Accord’s policy and MN Statutes.
  • Represent the agency to the people we support, our community partners and external constituents.
  • Serve as a positive role model for other employees.
  • Meet completion deadlines for all required and assigned training related to the position.
  • Comply with all federal and state laws, regulations and administrative rules that apply to program areas.
  • Travel throughout St. Louis County and surrounding counties to job sites and meeting locations; limited overnight travel possible.
  • Perform other duties as assigned.
