About The Position

The Care Coordinator role involves managing patient referrals, coordinating nursing care, and ensuring compliance with agency standards. This position requires excellent communication and organizational skills to act as a liaison between patients, nurses, pharmacies, and physicians. The Care Coordinator will utilize technology and software to manage patient information, track lab results, and document all communications. This role also involves participating in process improvement meetings and upholding patient confidentiality.

Responsibilities

  • Respond efficiently and professionally to pharmacies regarding patient referral inquiries about availability and scope of services.
  • Demonstrate excellent judgment and decision-making skills to select the appropriate nurse for individual patient needs based on location, therapy, and special considerations.
  • Ensure all information and required documentation is received to provide patient care in compliance with agency standards.
  • Ensure assigned nurse possesses the required skills and knowledge for individual patient therapies by verifying training and/or scheduling training calls and providing training resources.
  • Communicate with nursing staff regarding scope of care, scheduling, and required charting for each individual patient.
  • Serve as a liaison between patient, nurse, and following pharmacy and/or physician to communicate and advocate for patient needs, questions, or concerns.
  • Maintain knowledge and demonstrate efficient use of all technology and software to properly chart confirmation of visit coverage, future coverage needs, patient-specific special requirements, lab locations, and order expirations.
  • Document details of all communications, concerns, and issues in patient charts promptly.
  • Attend weekly meetings regarding process improvements, patient growth, and team efficiency.
  • Ensure all ordered labs are obtained correctly by the assigned nurse and properly input data to guarantee the results are obtained and provided to the following physician and pharmacy in a timely manner.
  • Maintain knowledge of, and effectively practice all patient confidentiality and ethical standards.
  • Promptly report issues or concerns regarding patient care to Clinical Supervisor and Coordination Liaison.
