About The Position

The Care Coordinator is responsible for providing and coordinating resident care to ensure quality care is delivered. This role also involves providing care team leadership. The position requires the individual to be responsible for the safety of residents in the case of emergency, serve as a 'go-to' resource person for team members, and participate in the orientation and training for care team members. The Care Coordinator fosters a positive work environment and provides strong support for developing skills with other care team members. They review, update, and provide resident care as outlined in resident service plans, perform rounds with physicians, and communicate new orders to staff and family as required. This role also assists with schedule coverage needs, answers resident call signals, administers/passes medications, reviews and processes physician orders using eMAR, and assists with resident evaluations. The Care Coordinator conducts departmental reviews/audits and provides results to the Director of Resident Care. They ensure adequate medication and supply inventory, observe and report non-emergent resident behavior and condition changes, and complete daily charting, documentation, and reporting. Participation in or leading activities as planned by Life Enrichment is also a key function, along with assisting and reminding residents to participate in activities. The role requires compliance with infection control, safety, emergency, and confidentiality policies and procedures, and participation in the on-call rotation as assigned. The Care Coordinator promotes positive relationships with residents, staff, and visitors, remains awake and alert while on duty, and performs any other duties as requested within the scope of responsibility and requirements of the job. Requirements

  • High school diploma/GED.
  • Successful completion of Care Specialist I and II curriculum within 90 days of hire.
  • At least 6 months of recent experience as a Care Specialist or equivalent experience.
  • CPR/First Aid certification or obtain within thirty (30) days of hire.
  • Demonstrable knowledge and experience using Microsoft Office in a Windows environment.
  • Capacity to master company specific software.
  • Ability to multi-task, prioritize and work with minimal supervision.
  • Effective oral and written communication skills.
  • Professional telephone etiquette and customer service skills.
  • Superior communication and interpersonal skills.
  • Communicate in a respectful and professional manner with residents, families, co-workers and leadership staff at the community.

Responsibilities
