About The Position

The Care Coordinator will provide intensive care coordination activities for members, ensuring they have access to, engage in, and retain needed services as defined in their care plan. These services may encompass acute medical care, primary medical care, preventative medical services, home health care, behavioral health services, community social support, housing, and state and federal entitlements. The role involves advocating for member services, conducting home visits, participating in case conferences and trainings, and maintaining relationships with service providers. Accurate and timely documentation, including member charts, records, and billing logs, is essential. The Care Coordinator will also attend supervision with the Program Director as per agency policy.

Requirements

  • High School Diploma or GED
  • 2 years of care management experience
  • Experience working with the homeless populations required
  • Experience working with HIV populations required
  • Good verbal skills in Spanish
  • Exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues
  • Clean valid driver's license required

Responsibilities

  • Provide intensive care coordination activities for members.
  • Assure that member has access to, engages in and retains needed services as defined in the member’s care plan. Such services may include: Acute Medical Care, Primary Medical Care, Preventative medical care Services, Home Health Care, Behavioral Health Services, Community Social Support Services, Housing & State and Federal entitlements.
  • Advocate for member services by phone and in person as needed.
  • Perform home visits and follow-up to monitor services and member status.
  • Participate in case conferences and trainings.
  • Maintain relationships with service providers.
  • Maintain accurate and timely documentation including all documents included in member charts, records and billing activity logs.
  • Attend supervision with Program Director as per agency policy.
