About The Position
Segue Health, a faith-based medical practice, is seeking a compassionate Care Coordinator to join our team serving patients in the Fort Worth, TX area. This role focuses on coordinating post-discharge transitional care management. The position requires a blend of clinical knowledge, strong organizational skills, and excellent communication abilities.
Requirements
- Experience in the medical field.
- Knowledge of Microsoft Excel.
- Ability to learn EMR/EHR software.
- Must have transportation to and from local hospitals.
- Must have a heart to serve others.
- Attention to detail and organizational skills are MOST important.
Nice To Haves
- Experience in social work, nursing, and/or home health.
Responsibilities
- Working with referring facilities to generate a list of patients eligible for Transitional Care Management (TCM) services.
- Visiting referrals while in the facility to explain the TCM program and identify anticipated post-discharge needs.
- Daily logging of referrals and discharges.
- Creating charts in EMR and uploading face sheets, H&Ps, and Discharge Summaries.
- Making post-discharge calls to patients within 24 hours.
- Scheduling patients, preferably prior to discharge, for in-home visits.
