We are seeking a compassionate and dynamic individual to join our team as a Care Coordinator for the PIVOT program. In this role, you will have the opportunity to coordinate care for our patients, making a meaningful difference in their lives and supporting them on their journey to recovery. Please note that all applicants must undergo a pre-employment drug screening, which includes testing for marijuana, as well as a comprehensive background check and TB Test.

Requirements

  • A degree in a behavioral health-related field, such as an associate or bachelor’s degree, is preferred.
  • Obtain the Certificate of Addiction Technician (CAT) within 18 months from the hire date.
  • Obtain case management certification within 90 days of hire.
  • Minimum of two years of experience working in the substance misuse and mental health field.
  • Must have a good understanding of substance misuse, mental health, and treatment delivery systems.
  • Proficiency in advanced computer skills is required.
  • CPR and First-Aid certification (the agency covers the costs for both initial training and renewal courses).
  • Must be able to take vital signs, recognize possible medical problems, and refer to the medical staff when necessary.
  • Valid driver's license and good driving record (must provide a current, unofficial DMV driving record upon hire).
  • Effectively communicate within a multidimensional treatment program that includes medical, psychiatric, and substance use-related components.

Responsibilities

  • Responsible for the development of care planning and patient needs assessments.
  • Facilitates life skills groups in accordance with the CTP policies and procedures.
  • Acts as liaison, establishing networks and collaboration with hospitals, community health centers, and state and human service agencies about patient services and needs.
  • Document admissions, care management needs, discharge, and discharge planning.
  • Administer case management assessments to identify patient needs and ensure the Addiction Severity Index (ASI) is completed at intake.
  • Organize a schedule of patient bed dates, transitioning levels of care, and consistent communication with the Medicaid Placement Coordinators.
  • Assists treatment team with patient intake, transfer, and discharge assessments.
  • Collaborating with the facility treatment team (medical, psychiatric, and SUD) to identify patients’ needs.
  • Assist patients in meeting their basic needs, such as clothing, medical care, legal support, and religious services.
