Under the supervision of the Program Director, the Care Coordinator is responsible for anything related to the coordination of care for all ROWI clients. The Care Coordinator will coordinate individualized services including orientation, transportation, doctor visits, and schedules. This role ensures programs and services are implemented at the highest standards and clients receive the highest level of care. The Care Coordinator is responsible for client supervision and ensuring they attend designated groups throughout programming. They will also facilitate assigned groups and activities, conduct client orientation, and ensure compliance with Joint Commission Standards. Documentation, transportation coordination, and client lunch coordination are key aspects of this role, alongside serving as a role model and maintaining confidentiality.
