This is a full-time, non-exempt position funded by the CCBHC grant, ending September 29th, 2027. The Care Coordinator will build rapport and engage effectively with individuals from diverse backgrounds using a warm, compassionate, and person-centered approach. This role requires strong interpersonal and communication skills to establish trusting relationships, provide exceptional customer service, and maintain professionalism in a fast-paced behavioral health environment. The Care Coordinator is responsible for client access to care triage, screening, and scheduling into services. They will ensure client care is coordinated between the client and all healthcare providers, collaborating with local agencies for continuity of care. This role also ensures clients have access to needed services during transitions between levels of care, provides education and communication about physical and behavioral health status, and develops strategies with clients to achieve their healthcare goals. The Care Coordinator will provide encouragement and support to help clients overcome difficulties in managing their health, remove barriers, and achieve optimal health outcomes. They will assist with internal and external warm hand-offs, enter specific CCBHC data into electronic health records and other databases, and track/report client outcomes. Documentation must be completed as required and maintained in accordance with Many Rivers policies. Effective communication with supervisors, therapists, medical providers, and other team members is crucial for quality continuity of care. Participation in in-service trainings, completion of assigned trainings, and attendance at required meetings are also part of the role.
