About The Position

Morrison Center is seeking an Adult Care Coordinator for our Brewer, Maine location. Candidates should be dedicated, compassionate professionals who are passionate about empowering adults with behavioral health, intellectual and developmental disabilities. Experience working with Behavioral Health Home Services is preferred. $1,000 sign-on bonus to be paid with first paycheck.

Requirements

  • Bachelor’s degree in a related field
  • MHRT/C, provisional, or eligible to apply
  • A minimum 1 year of paid experience working with individuals with behavioral health, intellectual and/or developmental disabilities (e.g., DSP, BHP, RBT, etc.)
  • Strong organizational, writing, and time management skills
  • Ability to thrive in a fast-paced environment with multitasking and problem-solving responsibilities

Nice To Haves

  • Experience working with Behavioral Health Home Services is preferred.
  • Previous experience in care coordination is preferred but not required

Responsibilities

  • Provide comprehensive care coordination services to adults enrolled in the Behavioral Health Home program.
  • Maintain an active caseload and monthly contact with members.
  • Conduct home visits, community visits, and office-based appointments as needed to support member engagement and service delivery.
  • Facilitate regular interdisciplinary team meetings and coordinate communication among providers, supports, and community partners.
  • Complete required assessments, including comprehensive assessments, person-centered assessments, annual updates, and 90-day reviews in accordance with DHHS and Behavioral Health Home requirements.
  • Develop, implement, monitor, and update individualized care plans using a person-centered and recovery-oriented approach.
  • Coordinate physical health, behavioral health, long-term support, and community-based services to promote whole-person wellness.
  • Assist members in accessing healthcare services, housing resources, employment supports, benefits, transportation, and other community resources.
  • Monitor member progress toward identified goals and adjust care planning strategies as needed.
  • Advocate for members and support informed decision-making, self-advocacy, and increased independence.
  • Identify gaps in services and collaborate with providers and community partners to address barriers to care.
  • Maintain accurate, timely, and compliant documentation in accordance with DHHS, MaineCare, Behavioral Health Home, and Morrison Center requirements.
  • Participate in quality improvement activities, audits, and program compliance initiatives.
  • Serve as a professional liaison among members, families, healthcare providers, behavioral health providers, and community agencies.
  • Represent Morrison Center with professionalism, compassion, cultural humility, and a commitment to recovery-oriented care.
  • Travel within the community as necessary to meet with members, attend appointments, and support coordination of services.
