Under the supervision of the Program Director, the Care Coordinator is responsible for all aspects of coordinating care for ROWI clients. This includes coordinating individualized services such as orientation, transportation, doctor visits, and scheduling. The role ensures programs and services are implemented at the highest standards, clients receive the highest level of care, and are supervised to ensure attendance at designated groups. The Care Coordinator will also facilitate assigned groups and activities, conduct client orientations, administer screenings and assessments, and ensure compliance with Joint Commission Standards. Documentation in the EMR system is a key responsibility, including shift reports, transportation logs, group activity documentation, and Urine Analysis administration. The role also involves coordinating daily client lunches and serving as a positive role model while maintaining client confidentiality and complying with the dress code. Completing tasks accurately and timely, attending meetings, and managing client boundaries and supervision are also essential functions. All EMR documentation must be submitted within 24 hours.
