Under the supervision of the Program Director, the Care Coordinator is responsible for all aspects of care coordination for ROWI clients. This includes coordinating individualized services such as orientation, transportation, and doctor's appointments, as well as managing schedules. The role ensures that programs and services are implemented to the highest standards, providing clients with the best possible care. The Care Coordinator is also responsible for client supervision, ensuring attendance at designated groups, facilitating assigned groups and activities, and conducting client orientations. This includes initial assessments like UA screening and PHQ9. The position requires ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, and driving clients as needed. All transportation and group activity documentation must be entered into the EMR within 24 hours. The role also involves administering and submitting documentation for client Urine Analysis, assisting with facility cleanliness, coordinating daily client lunches, and serving as a positive role model. Maintaining client confidentiality, complying with the dress code, completing tasks accurately and on time, and attending staff meetings are also key responsibilities. The Care Coordinator must be able to maintain clear boundaries, set limits, manage clients needing direct supervision, and interact professionally with clients and staff.
