About The Position

Care Coordinators serve as the central entry point and operational backbone for the Care Management department. They provide comprehensive care coordination for members, including those in Complex Care Management (CCM), Readmission Prevention (RAP), and those who do not fit into existing highly specialized programming. By taking ownership of administrative tasks, managing post-discharge transportation needs, and resolving inbound inquiries with a focus on one-call resolution, the Care Coordination Team allows our RN Care Managers to operate at the top of their license and helps members reach their optimum health goals in the most cost effective way. This is a remote position, open to candidates who reside in: Arizona; Florida; Georgia and Texas. While your daily work will be completed from your home office, occasional travel may be required for team meetings and company events.

Requirements

  • 1+ years of experience with care coordination and navigation in a healthcare setting.
  • Proficient computer literacy skills, with a strong aptitude for using EMRs (like Medecision) and telephony systems (like InContact) for seamless workflows.
  • Excellent interpersonal communication skills including exceptional listening skills.

Responsibilities

  • Be the primary bridge and entry point for all members and providers working with the Care Management team.
  • Field inbound phone calls from members and providers, using optimized IVR pathways to ensure accurate routing and striving for one-call resolution.
  • Manage care coordination for members across various programs, including CCM, TOC, and unassigned populations, across a variety of settings (hospitals, outpatient clinics, home care, supply vendors, dialysis) while maintaining strict confidentiality and the highest level of professionalism.
  • Manage non-emergent medical transportation (NEMT) via UberHealth for members, strictly following program guardrails to decrease readmissions and improve health outcomes.
  • Handle internal escalations to specialized departments such as Claims, Eligibility, and Care Navigation.
  • Lead special projects as subject matter experts.
  • Collaborate cross-departmentally to create a positive member experience.
  • Perform all other tasks or additional responsibilities.
