The Care Coordinator is responsible for the coordination of care for all ROWI clients. This role involves coordinating individualized services including orientation, transportation, doctor visits, and scheduling. The Care Coordinator ensures programs and services are implemented at the highest standards and clients receive the highest level of care. They are also responsible for client supervision, ensuring clients attend designated groups, and facilitating assigned groups and activities. The role includes conducting client orientations, administering assessments, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, and driving clients as needed. Additionally, the Care Coordinator documents all transportation and group activity in the EMR, administers and submits documentation for client Urine Analysis, coordinates client lunch daily, and serves as a role model to treatment staff, clients, employees, and guests. Maintaining confidentiality, complying with the dress code, completing tasks accurately and timely, and attending staff meetings are also key responsibilities. The role requires the ability to maintain clear boundaries, set limits, manage clients needing direct supervision, and interact professionally. All EMR documentation must be submitted within 24 hours.
