About The Position

Segue Health, a faith-based medical practice, is seeking a Care Coordinator to work in the Crossville, TN area. This role focuses on coordinating post-discharge transitional care management. Responsibilities

  • Work with referring facilities to generate lists of patients eligible for Transitional Care Management (TCM) services.
  • Visit referrals in the facility to explain the TCM program and identify anticipated post-discharge needs.
  • Log referrals and discharges daily.
  • Create charts in EMR and upload face sheets, History & Physicals (H&P), and Discharge Summaries.
  • Make post-discharge calls to patients within 24 hours.
  • Schedule patients for in-home visits.
