The Care Coordinator is responsible for the coordination of care for all ROWI clients. This role involves coordinating individualized services including orientation, transportation, doctor visits, and scheduling. The Care Coordinator ensures programs and services are implemented at the highest standards and clients receive the highest level of care. They are also responsible for client supervision, ensuring clients attend designated groups, and facilitating assigned groups and activities. The role includes conducting client orientations, administering assessments, ensuring compliance with Joint Commission Standards, documenting shift reports, coordinating transportation schedules, administering Urine Analysis, and assisting with facility cleanliness and client lunch coordination. The Care Coordinator serves as a role model, maintains confidentiality, complies with the dress code, and completes tasks in a timely and accurate manner. They must also attend staff meetings, maintain clear boundaries, and ensure all EMR documentation is submitted within 24 hours.
