About The Position

Care Coordinators in the Outpatient Department support individuals and families accessing Kanza’s behavioral health services by managing intake, referrals, follow-ups, and ongoing care coordination. They help clients navigate both Kanza’s programs and external resources, ensuring integrated, person- or family-centered care. Coordinators maintain a caseload, facilitate communication among providers, and exchange confidential information to support seamless treatment. Working closely with a multidisciplinary team, their goal is to enhance client engagement and improve quality of life outcomes.

Requirements

  • High School Diploma or equivalent required
  • At least two years of applicable work experience that supports the skills and requirements of the position
  • Strong organizational, time management, communication and interpersonal skills.
  • Excellent communication and interpersonal skills.
  • Intermediate computer skills and proficiency required

Nice To Haves

  • Associate’s degree preferred but not required
  • Bilingual skills are a plus, but not required

Responsibilities

  • Provide warm and friendly customer service in person, by phone, and in writing to individuals and families receiving services from Kanza, as well as to Kanza and external care providers to facilitate ongoing care coordination, collaboration, and communication.
  • Understand and follow HIPAA and 42 CFR Privacy Laws for patients with mental health and substance use disorders to ensure confidentiality and privacy.
  • Develop and maintain a positive working relationship with Kanza’s care providers, as well as community providers to ensure good coordination and care for patients.
  • Assist individuals and families entering Kanza’s services with the intake process and paperwork, including appropriate behavioral health and physical health screeners, patient consents, releases of information, and referrals.
  • Complete administrative work in a timely and accurate manner, most of which will occur in the patient electronic health record. This includes proper documentation of all contacts and activities pertaining to care coordination in the electronic health record system.
  • Link individuals and families with needed resources and services according to the treatment plan and regularly monitor and ensure that services are meeting their needs.
  • Completes comprehensive surveys with individuals at the appropriate frequency depending on the survey type. Responsible for tracking and administering surveys as well as data entry.
  • Support individuals in completion of applications for applicable benefits to include KanCare Medicaid, non-emergency transportation resources, and other social services.
  • Utilize supervisory supports appropriately and participates in all team/program meetings.
  • Complete annual and assigned trainings on time and participate in ongoing professional development to enhance the care and services provided, including recovery-oriented, trauma-informed, person- and family-centered care.

  • Manage multi-task situations and conflicting demands, cope with stressful situations, prioritize projects and responsibilities, and meet critical deadlines.
