The Care Coordinator is responsible for all aspects of coordinating care for ROWI clients. This includes managing individualized services such as orientation, transportation, and doctor's appointments. The role ensures programs and services meet the highest standards, and clients receive optimal care. The Care Coordinator also facilitates groups, conducts client orientations, and ensures compliance with Joint Commission Standards. They are responsible for documenting shift reports, coordinating transportation schedules, administering and documenting client Urine Analysis, and coordinating client lunches. The position requires serving as a role model, maintaining client confidentiality, and completing tasks accurately and on time. Additionally, the Care Coordinator must maintain clear boundaries, interact professionally with clients and staff, and ensure all EMR documentation is submitted within 24 hours.
